Apeks XL4, XL4+ & Ocea 2nd Stages/Octopus Safety Recall (July 1, 2026)

Check your regulator's serial number to see if it's affected, or bring it to us to check. Products included in this recall have serial numbers ranging from: 250402212 to 260402893 and were manufactured between April 2025 and April 2026.

A defect in the spindle mould tool has allowed the spindle assembly to be installed incorrectly. Under demanding diving conditions, particularly at depths exceeding 45 meters, this can restrict the flow of breathing gas, posing a risk of inadequate air supply to the diver at depth.

At this time, we do not have repair kits from Apeks. If you bought an affected regulator from us or are in the Edmonton-area, please contact us with your name, phone number and email and we will be in touch as soon as we receive the parts kits.

Hollis 200LX Second Stage Regulator - STOP USE (February 11, 2026)

Affected Purchase Window: Units purchased between May 2, 2017, and May 15, 2025, may be affected. 

IF YOU OWN THIS REGULATOR:

  • Do not dive with the affected regulator.
  • Remove it from service immediately.
  • Do not attempt to repair, modify, or continue using the product.

Huish Outdoors has received reports of internal components within certain Hollis 200LX second stage regulators separating or fracturing before or after a dive.

Our investigation indicates that some units may exhibit reduced impact resistance, particularly if the second stage is dropped or struck on the adjustable inhalation knob. In these cases, damage to the inlet tube assembly could render the second stage non-functional. 

While no incidents have been reported during an active dive, this condition could pose a drowning hazard if it were to occur in use. As a precaution, we are issuing this Stop Use notice while we complete our technical review and prepare a corrective remedy. 

Huish Outdoors is finalizing:

  • The required replacement parts, and
  • A standardized retrofit procedure to be performed by authorized technicians.
